fill it out and mail this form to the postal address listed on the form.
If we need information in addition to what is on this form, we will call
or write you. We will respond as quickly as possible. However, please
be aware that our limited resources mean that we can accept very few cases.
Also, because there may be some delay in an initial response, you should
seek advice from an attorney about time deadlines that might apply to
your case. Do not wait to hear from us before taking action with regard
to your case, or finding out about time deadlines that might be applicable.
We cannot give you any advice about your case, and you are responsible
for meeting all deadlines.

Because we receive so many requests for assistance, we cannot return any
materials that you submit to us with your intake questionnaire.
